The Elementary Application is available for download here.  Elementary grade levels include kindergarten through fifth grade.  The list of available elementary schools is on the reverse of the application.  Please be sure to completely fill out your application.  False, misleading, or incomplete information will automatically disqualify your child from the CVS Choice Program.  Applications are being accepted now.  Please indicate your top three choices for elementary school on your application.  

The Secondary Application is available for download here.  Secondary grade levels include sixth through twelfth grade.  The list of available secondary schools is on the reverse of the application.  Please be sure to completely fill out your application.  False, misleading, or incomplete information will automatically disqualify your child from the CVS Choice Program.  Applications are being accepted now.

Applications can be dropped off during normal business hours (7:30am - 4:30pm) or mailed to 19120 Cass Avenue, Clinton Twp., MI  48038.
